L’ufficiale di polizia di New York, Ralph Sarchie (Eric Bana), che lotta con i suoi problemi personali, indaga una serie di crimini inquietanti e inspiegabili. Si unisce ad un prete insolito (Edgar Ramírez), che ha studiato i riti di esorcismo, per combattere i casi di possessione demoniaca terrificanti che terrorizzano la città. Ispirato al libro che racconta i dettagli dei casi agghiaccianti e realmente accaduti descritti da Sarchie.
